*A family discount is provided to families with three or more children enrolled in KYSA soccer.
The discount is $50.00 off the total registration cost. Payments can be made by cheque, money order, cash,
Interact, Visa or MasterCard. All cheques or money orders should be made payable to the Kamloops Youth
Soccer Association. NSF cheques are subject to a $25.00 service charge. Any players registering after February 1
will be placed on a team only if space is available. There is no guarantee that the player will be placed on a
specific team after February 1!

KYSA takes no responsibility for any lost or misplaced faxed or mailed forms. All forms must be clearly legible
and include payment. Completed registration forms and payment must be received in the KYSA office no later
than the close of business Monday, February 1, 2011 to avoid late registration penalties.

Players must register through the KYSA office to be placed on a team, not through a team’s coach.
Registrations can be hand delivered to the KYSA office at 1550 Island Parkway Drive (Beneath the
Bread Garden Restaurant) on McArthur Island. Office hours are 9:00am to 4:00pm, Tuesday through Friday.

A cancellation fee of $25.00 will be enforced for any cancellations after registration is accepted regardless of
the date registered during the registration period. There will be no refunds given after April 15, 2011 except in
cases where a serious injury forces the player to withdraw or medical conditions prevail that prevent a child from
continuing in soccer. In this instance, a doctor’s note may be requested.
